NHS Credit Union Membership

Application for the NHS Credit Union is open to NHS Employees in Scotland, North England (North East, North West and Yorkshire & Humberside) and their families living at the same address.

All applicants must confirm that they have read the terms and conditions and agree to abide by them. The terms and conditions can be accessed HERE.

All applicants must also confirm that they have downloaded, read and understood the UK FSCS information sheet and exclusions list. These documents can be downloaded ClickHERE

After reading the terms and conditions, and FSCS information sheets, please read the two statements below to see which applies to you - and PLEASE ensure you click the right statement to make certain that you complete the appropriate application form.
When we receive the completed application, we will send you either a payroll deduction form or a direct debit mandate. This must be signed and posted back to us to allow us to complete your application and take the appropriate deductions.

Membership Application with payroll deduction
I work for one of the following NHS Health Boards, agree to abide by the terms and conditions, and wish my contributions taken by payroll deduction.

In order for payments to be made into your account we require an original signature from you to send to your pay office. To do this you have to complete a payroll deduction mandate. Once you have completed and submitted your application form we will print it off and return this part to you for your signature.


Membership Application with Direct Debit Mandate
I work for another authority in Scotland, North East England, North West England or Yorkshire and the Humber and wish my contributions taken by Direct Debit.

In order for payments to be taken by Direct Debit we require a mandate with an original signature from yourself to send to your bank.

Once you have completed your application form we will send you a Direct Debit mandate to you and we would be obliged if you would complete, sign & return it as soon as possible back to us.
